Request for ensuring accessibility
How to submit a request?
- Download the appropriate form available in the files for download or use your own form.
- Complete the form and remember that it must contain the mandatory elements marked with an asterisk (*).
- Read the GDPR information clause. It can be found in the footer of our website under the GDPR tab and the RODO US Kielce subpage.
- Submit the completed request to the Office in one of the following ways:
- by post to the following address:

- you can submit the request in person at the headquarters of the Statistical Office in Kielce or at the Sandomierz Branch;

Requests for ensuring accessibility and communication will be considered without undue delay, and in the case of:
- digital accessibility – no later than within 7 days from the date of submission (receipt) of the request by the Office,
- architectural or information and communication accessibility – no later than within 14 days from the date of submission (receipt) of the request by the Office.
How to submit a complaint?
If you have submitted a request and we fail to ensure accessibility for you or fail to do so within the prescribed time limit, you have the right to submit a complaint concerning the lack of accessibility.
A complaint concerning the lack of architectural or information and communication accessibility may be submitted to the President of the Management Board of the State Fund for Rehabilitation of Persons with Disabilities (PFRON), while a complaint concerning the lack of digital accessibility may be submitted to our Office in the same way as a request.
Complaints submitted to our Office will be considered without undue delay, but no later than within one month from the date of submission (receipt) of the request by the Office.
Remember:
A request for ensuring digital accessibility or a complaint concerning the lack of accessibility may be submitted by anyone.
A request for ensuring architectural or information and communication accessibility, or a complaint concerning the lack of accessibility, may be submitted by a person with special needs or their statutory representative, upon demonstrating a factual interest, i.e. indicating a genuine need to use the services of the Office.
A request for ensuring communication with the Office may be submitted by a person with special needs or their statutory representative.

We are open from Monday to Friday, 7:00 a.m. to 3:00 p.m.
The Director of our Office receives persons regarding requests and complaints on Mondays from 2:00 p.m. to 4:00 p.m.
